Community Health Center, Inc. National Training and Technical Assistance Partners (NTTAP) Community of Practice (CoP) Application 2025-2026

Overview

Program Information

Funded by the Health Resources and Services Administration (HRSA) as the organization responsible for the National Training and Technical Assistance Partners (NTTAP) on Clinical Workforce Development, Community Health Center, Inc. (CHCI) under Moses/Weitzman Health System, Inc. (MWHS) delivers free technical and training assistance (T/TA) to health centers across the United States. The following application will provide you with a detailed introduction and overview of these free learning opportunities designed to move health centers from planning to implementation of replicable models. We welcome your organization to select and apply for the Communities of Practice (CoP) that best meet the needs of your strategic goals. Once the application is complete, your health center will be contacted to set up a Zoom call to review the expectations and syllabus of the CoP, as well as ensure the opportunity is a right fit for your health center.

On behalf of the Community Health Center, Inc., thank you for showing interest in participating in one or more of our upcoming Communities of Practice (CoP):  

  • Postgraduate Nurse Practitioner (NP) and/or Physician Associate (PA) Training Programs: This six session series will equip health centers and look-alikes with the knowledge and resources to launch their own Postgraduate NP and/or PA Training Program. This learning opportunity provides health center participants with a roadmap and support on how to implement the formal training program, including program drivers, program structure, recruiting, curriculum development, evaluation, and program accreditation.
    • Application Due: Friday September 26th, 2025
    • CoP Start Date: Tuesday October 14th, 2025
  • Comprehensive and Team-Based Care: This eight session series will support health centers and look-alikes with developing highly trained clinical primary care teams to move towards team-based comprehensive care and improving at least one UDS measure. The learning opportunity provides health center participants with quality improvement concepts and skills to systematically achieve a specific aim, and identify areas for process improvement and role optimization. 
    • Application Due: Friday October 24th, 2025
    • CoP Start Date: Wednesday November 5th, 2025

Learning Objectives

Upon completion of this activity, participants should be able to do the following:

  • Use assessments of their health center to identify areas for process improvement
  • Utilize session content, one-on-one support, and deliverables to move from planning to implementation of replicable models
  • Contribute to the learning among participating practice teams by engaging in Communities of Practice (CoP) activities
